VFC Provider Profile Report (Alaska)
This report lists the annual VFC Enrollment Provider Profile form for a specific data range, Organization (IRMS), Facility, and/or VFC PIN. It includes both VFC eligible and ineligible vaccinations by age group (<1 year old, 1-2 years, 3-6 years, 7-18 years, 19+ years, and a total).
This report can be scheduled.
To run this report, click the Reports > State Reports > Other > VFC Provider Profile Report link. The VFC Provider Profile Report page opens. Enter the criteria and click Create Report. The report displays in a separate browser tab and can be printed using the browser's print function. To export the report, click Export Report.
The available report criteria fields are as follows:

Vaccine Date Range |
Enter the From and Through dates for the vaccine data range. |

Organization (IRMS) |
To limit the report by an Organization (IRMS) or Group, select it from the related drop-down list, after which the radio button is automatically selected. Do Not Limit is the default. |

Facility |
To limit the report to a specific Facility, select it from the drop-down list, after which the radio button is automatically selected. Do Not Limit is the default. |
|
VFC PIN |
To limit the report to a specific VFC PIN, select it from the drop-down list (after which the radio button is automatically selected). |
