School Immunization Report, First Time Enterer
This report displays a breakdown of grade level, school type (public or non-public), enrollment levels, complete records (no shots are needed for school entry), incomplete records (shots are needed), vaccinations needed, and those who are exempt, with subtotals per grade and totals at the bottom.
If a school does not have any of the grades that the report is being run for, the school does not display in the search results. If a school has the grades, the report runs even if the totals are all zeros. If a school has a subset of the grades (i.e., only one of the grades but not all of them), the grade displays even if it is all zeros, but for the grades it does not have under enrollment, it displays "No Grade [level of grade]."
Run the Report
To run this report, School Nurse users can click the Reports > School Reports > School Immunization Report, First Time Enterer link. The School Immunization Report, First Time Enterer page opens. Enter the report criteria and click Create Report. Click Export to create a downloadable version of the report.
The available fields on this page are as follows:

Enter the school name. This field is required. |

Included Grade Levels |
Select All (default) to include all grade levels, or select one or more grades in the list, after which the radio button is automatically selected. This field is required. |

Vaccination Details By |
Select either Needing Vaccination (default) or Completed Vaccination. This field is required. |

Series |
Select a series from the drop-down list. This field is required. |
