School Immunization Report
This report displays a breakdown of grade level, district/region, patient borough/census area, school district, school type (public or non-public), grade levels, complete records (no shots are needed for school entry), incomplete records (shots are needed), vaccinations needed, and those who are exempt, with subtotals per grade and totals at the bottom.
If a school does not have any of the grades that the report is being run for, the school does not display in the search results. If a school has the grades, the report runs even if the totals are all zeros.
Run the Report
To run this report, School Nurse users can click the Reports > School Reports > School Immunization Report link. The School Immunization Report page opens. Enter the report criteria and click Create Report. Click Export to create a downloadable version of the report.
This report can be scheduled.
The available fields on this page are as follows:

Grade Level (default) |
This is a default setting. |

District / Region |
To limit the report to a specific district or region, select it from the drop-down list, after which the option is automatically selected. |

County / Parish (Patient Borough / Census Area) |
To limit the report to a specific county or parish, select it from the drop-down list (after which the option is automatically selected). |

School District |
To limit the report to a specific school district, select it from the drop-down list, after which the option is automatically selected. |

School |
To limit the report to a specific school, click the Click to select link, search for the school in the pop-up window, and select it. |
|
School Type |
To limit the report to a school type, select it from the drop-down list. All is the default. |

Included Grade Levels |
Select All (default) to include all grade levels, or select one or more grades in the list, after which the radio button is automatically selected. |

Vaccination Details By |
Select either Needing Vaccination (default) or Completed Vaccination. This field is required. |
|
Series |
Select a series from the drop-down list. This field is required. |
