Exemption Aggregate Report (West Virginia)
This report lists the schools and vaccines for medical exemptions.
To run this report, authorized users can click the Reports > State Reports > Exemption > Aggregate Report link. The Exemption Aggregate Report Menu page opens. Enter the criteria and click Create Report. The report displays in a separate browser tab and can be printed using the browser's print function. To create a report for export, click Export Report.
The available report criteria are as follows:

Zip Code |
To limit the report to a specific zip code, enter it in this field. |

School |
To limit the report to a specific school, click the Click to select link, search for the school, and click the arrow button in the Select column for the school. |

Vaccine |
To limit the report to a specific vaccine, select it from the drop-down list. |

Sort By |
To sort the report by a specific field/column, select it from the drop-down list. Examples:
