Exemption Detail Report (West Virginia)
This report lists the patients and vaccines for medical exemptions.
Run the Report
To run this report, authorized users can click the Reports > State Reports > Exemption > Detail Report link. The Exemption Detail Report Menu page opens. Enter the criteria and click Create Report. The report displays in a separate browser tab and can be printed using the browser's print function. To create a report for export, click Export Report.
The available report criteria are as follows:

To limit the report by status, select Active, Denied, or Submitted from the drop-down menu. |

Vaccine |
To limit the report to a specific vaccine, select it from the drop-down list. |

Sort By |
To sort the report by a specific field/column, select it from the drop-down list. Examples:
